Mountain State Home Health Care can assist you in living independently in your own home.
We offer personal care services, environmental assistance, and companionship to assist aged and disabled individuals in living at home.

• Cost to the individual is less than Nursing Home placement

• Individuals receive quality care in their homes rather than an institution

• Family and friends are allowed to actively participate in individuals care and environmental management

• Individuals remain independent longer and actively participate in care planning

• Individuals are allowed the Freedom of Choice to select care providers

• Managed Care by qualified Registered Professional Nurse teams work together for the benefit of the individual

• Certified Homemakers (CHM) and Certified Nursing Assistants (CNA) providing quality care for individual

• Individuals participate in planning their service hours and tasks that Homemakers will complete

• All care providers are supervised by Registered Professional Nurses

• All care providers receive on going in-service training to keep them up to date and informed

• All care providers are evaluated by Registered Professional Nurses and clients throughout the year for performance and quality of care

• Individuals have the freedom of choice to request their care provider

• Staffing Coordinators are available 24 hours a day seven days a week, including holidays to assure quality care providers are available when you need them

• Registered Professional Nurses will assist with care planning and instructing care providers with your specific needs and levels of care
